Patient Information

New Patients: Please print and complete the New Patient paperwork for your age.  Per our office policy, you must pay your co-payment at the time of your visit.  With the high volume of new patients, we will not be able to reschedule your visit that has not been canceled or rescheduled within 24 hours before your visit.

For children under the age of 18, please bring a copy of their current immunization record.

Please remember to bring the following to your appointment:

  • Your medical insurance cards
  • Your drivers license or a picture ID
  • Check, credit card or cash for your co pay
  • Completed New Patient paperwork

After Hours Assistance:  One of our providers can be reached after business hours for emergent questions.  The provider-on-call can be reached by calling our main number (363-5104) and following the prompts.

Open Access:  Our providers strive to see patients in 24 hours or less for all needs.  Because of this goal, they do not set aside dedicated phone time.  We respectfully request that all phone calls be routed through our staff.

Prescriptions:  We attempt to fill as many prescriptions electronically through a link with each participating pharmacy.  Refills requests also arrive via this link.  Please ask your pharmacist to send those requests a minimum of 48 hours prior to your need.

Payment:  Payment is expected at the time of service.  Our staff will bill Medicare, Medicaid and most private insurances on your behalf.  A full copy of the payment and collections policy is available upon request.  We also offer reduced rates to uninsured patients.  Please ask at the front desk for more information.

Hospital Admissions:  Our providers entrust the care of their patients to the Hospitalist Service of Marcus Daly Memorial Hospital here in Hamilton.

Nursing Home Admissions:  We provide on site care at the two local skilled nursing facilities in Hamilton.
